This message is definitely not normal.  Everything after this point does 
not matter; it is here where the trouble began.  It's coming from the 
lower layer USB core and indicates problems with the connectivity to the 
device or possibly protocol problems with the device itself.

I think the *only* time I've ever been able to force an error like that is 
by yanking the cable in mid-transaction (i.e. while it was in use) and 
then plugging it back in.  And even then the problem was rare.  My best 
theory at the time was that the device itself was in a crashed state due 
to being ripped out of the system previously and that because of this the 
USB interface had been "messed up".  But even that is a very weak theory 
because much of the USB protocol overhead is handled in the silicon of the 
FX2 microcontroller within the device.

Actually I think I managed to see this once before, when using a suspect 
USB cable.

In any case, this is still an indicator of problems :-(
