[pvrusb2] Updated fwextract.pl available

A new fwextract.pl is available.  It is directly downloadable here:

http://www.isely.net/downloads/fwextract.pl

This will also be in the next driver snapshot.  Changes include:

1. New image data added, thanks to Gary Buhrmaster (see yesterday) and 
Michael Burr (not on the mailing list).

2. Minor tweaks to allow operation in MS-${whatever} OS (also thanks to 
Michael).

Let me know if anyone sees any new problems.
