[pvrusb2] Persistent device nodes

Try the attached - uses the devices address.

Ttv at tv-desktop:~$ ls /dev/video-*
/dev/video-PVRUSB2-F0350180  /dev/video-PVRUSB2-F07E02FF
tv at tv-desktop:~$ ls /dev/dvb/
adapter0  adapter-PVRUSB2-F0350180
tv at tv-desktop:~$

It also handles 950q's if I plug them in.
