[pvrusb2] New driver snapshot: pvrusb2-mci-20060107

This driver snapshot rolls up pending patches.  The most significant 
change was new logic to tolerate a specified level of USB streaming 
errors.  The implementation normally leaves this off (i.e. "tolerance" set 
to zero), so it must be turned on via a module parameter in order to be 
effective.  I was only able to test this to the extent that in the "no 
error" scenario things continue to work fine.

Speaking of module parameters, they are all documented now on the main web 
page (look under "usage").

Oh, and now when firmware loading fails, the resulting behavior of the 
driver is no longer so misleading; in fact now it should even tell you if 
the firmware files specifically could not be located.
