[pvrusb2] help with usb id and making the driver to work

i'm trying to install a pvr usb2 on my machine. i was able to follow
the instruction in mike's website, and the compilation and
installation seems ok. i did not receive any error messages and the
module seemed to load fine. the firmware also installed in the correct
location.

however, it's not working. running "mplayer /dev/video0" said

File not found: '/dev/video0'
Failed to open /dev/video0

also, when i do lsusb, the usb id given is this:

Bus 001 Device 002: ID 04b4:8613 Cypress Semiconductor Corp. CY7C68013
EZ-USB FX2 USB 2.0 Development Kit

i'm using the pvrusb2-mci-20060607 version of the driver, linux kernel
2.6.15 (ubuntu dapper).
